Small Intestinal Bacterial Overgrowth (SIBO) Breath Test

Overview

The SIBO Breath Test is a non-invasive diagnostic tool that measures exhaled hydrogen (Hâ‚‚) and methane (CHâ‚„) gases after the patient consumes a lactulose solution. It is designed to detect:

  • Small Intestinal Bacterial Overgrowth (SIBO)
     
  • Intestinal Methanogen Overgrowth (IMO)
     

We offer both 2-hour and 3-hour testing profiles:

  • 2-hour – Standard assessment for most patients.
     
  • 3-hour – Recommended for patients with slower gut motility or constipation, offering extended insight. A flatline in the third hour may suggest hydrogen sulfide gas production.
     

 


Why Test for SIBO?

SIBO is caused by excessive bacterial growth in the small intestine, leading to nutrient malabsorption, inflammation, and a wide range of digestive and systemic symptoms.

How the Test Works

  1. Preparation – Special diet 24 hours before, overnight fasting, and avoidance of certain medications and supplements.
     
  2. Baseline Breath Sample – Collected before ingesting lactulose.
     
  3. Lactulose Solution – Patient drinks 10g lactulose in water.
     
  4. Breath Samples – Collected at regular intervals for 2–3 hours to measure hydrogen and methane gases.
     

Important: The test requires strict adherence to preparation guidelines for accurate results.

 


Patient Preparation Guidelines (Summary)

  • Follow a low-residue diet for 24 hours before the test (plain meat/fish, white rice, eggs, clear broth, oils, salt, pepper, black tea/coffee).
     
  • Fasting for 12 hours before the test – only plain water allowed.
     
  • Avoid antibiotics for 4 weeks, probiotics as advised, and laxatives or promotility agents for 7 days (if tolerated).
     
  • No gum, candy, smoking, or vigorous exercise on test day.
     
  • Certain medications may be continued if clinically necessary – consult your doctor before stopping any prescribed treatment.
     

 


Special Considerations

  • Lactulose intolerance/allergy – Not suitable for patients with confirmed allergy. Mild digestive discomfort may occur in lactose-intolerant individuals but does not affect results.
     
  • Diabetes – Use with caution as lactulose may affect blood sugar.
     
  • Pediatric patients (<25 lbs) – Test not recommended due to collection challenges and dietary restrictions.
